Nursing Services

The Collingswood and Oaklyn Public School Districts are committed to the health, wellness, and safety of all our students. The Nursing Health Services support your child’s educational experience. 

Health Office Update: Please complete the Health History Update form in Genesis for each student upon first login for the year.

The school nurse supports student success by providing health care through assessment, intervention, and follow-up for all children within the school setting. The school nurse addresses the physical, mental, emotional, and social health needs of students and supports their achievement in the learning process.

Physical Examinations & Immunizations Requirements:  

Yearly Physical Exam: performed within 365 days of the first day of school

Immunization Requirements: K-Gr 5 Immunization Requirements

*All updated forms due the day after your child's appointment in order to meet compliance*

Medication/Medical Procedure:

All medication must have a healthcare provider’s written order and written parent permission annually.

All medication must be brought to school in original packaging by a parent or guardian.

***All paperwork must be completed prior to any administration of medication/medical procedures.***
